1. You browsed to a website that no longer exists on Optus's network, but the Domain Name System host entry still points to our servers.
2. You browsed to the IP address instead of the name of a website hosted by Optus.
3. Your Internet browser is very, very old and doesn't send a "Host" header specifying the website you are trying to view. (Internet Explorer 3.0, Netscape Navigator 2.0, and later versions of both browsers, plus almost every other browser support the Host header, and it's unlikely that you're using one of these, so it's probably not this problem.)
